A lump where you injected: what the labels report
Reactions at the injection site are listed in these medicines' own labelling, and in the trials they were uncommon. What this page will not do is tell you what the lump on your abdomen is: nothing written in advance can, and the honest answer to a persistent hard area is that someone should look at it. What it can do is show you the numbers the labels actually report, and clear away a piece of research that keeps being applied here and does not belong.
What the trials counted
The labels report injection site reactions as a named side effect with a frequency attached:
- Ozempic® — in placebo-controlled trials, injection site reactions (given as injection-site discomfort and erythema) were reported in 0.2% of treated patients.
- Wegovy® — in the adult weight-reduction trials, 1.4% of treated patients reported an injection site reaction, against 1% of patients on placebo. The listed reactions were itching, redness, inflammation, hardening of the tissue and irritation.
That placebo figure is the part worth sitting with. One per cent of people injecting a medicine with nothing in it also reported a reaction at the site. Some of what gets noticed after an injection is the injection, not the drug, and neither number tells you which one yours is.
What rotation is, and what the label says it is for
The instruction itself is short and unambiguous. Wegovy's patient information says: "Change (rotate) your injection site with each injection. Do not use the same site for each injection." The prescribing information says the same thing in one line: rotate injection sites with each dose.
Notice what is not there. The labels tell you to rotate; they do not tell you it is to prevent lumps, and they do not promise that rotating stops a reaction happening. Anywhere you read a confident mechanism for that, check whether it came from the instructions or from someone filling in a gap. The research that keeps getting borrowed. Search for lumps at an injection site and most of what comes back is about lipohypertrophy — thickened fatty tissue that develops where a person has injected repeatedly. That body of work is about a different class of medicine: it describes a lipogenic effect specific to insulin, and the reviews measure it in people treated with insulin. GLP-1 receptor agonists are not that drug and do not have that action. Reading those findings across is not a small stretch, and this page will not make it.
What belongs in front of a clinician
A page cannot examine anything, so the useful line is not "this is fine" or "this is not" — it is which observations are worth taking to someone who can look:
- An area that stays hard, or that is still there weeks after the injection that made it.
- A lump that is growing, or that keeps returning in the same place.
- Redness that spreads, warmth, pus, or a fever: the signs that suggest an infection rather than a reaction.
- Skin that has changed texture across a whole area rather than at one point.
And separately from all of that: swelling of the face, lips, tongue or throat, or trouble breathing, is a possible allergic reaction and is an emergency: call your local emergency number (911 in the United States and Canada, 999 or 112 in the United Kingdom, 112 across the European Union and Türkiye).
What you can bring instead of a description
"It's been sore around there for a while" is hard to act on. Where each injection went, and when, turns the same conversation into a pattern someone can look at: whether the area in question is one you have been returning to, how often, and over what period. That is a record, not a diagnosis, but it is the part of the picture only you can supply.

Is a small lump after an injection normal?
Reactions at the site are a listed side effect of these medicines, and hardening of the tissue is one of the forms the Wegovy label names. That is not the same as a page telling you yours is nothing. Something that settles within a day or two is different from something still there next week, and the second one is worth showing.
Does rotating prevent it?
The labels instruct you to rotate. They do not claim it prevents reactions, and we are not going to claim it on their behalf. Follow the instruction because it is the instruction.
I read that repeated injections cause fatty lumps. Does that apply here?
What you read is almost certainly about lipohypertrophy, which is described in the research as an effect of a different medicine: one with a lipogenic action that GLP-1 receptor agonists do not have. It is the most-searched answer to this question and it is answering about another drug.
Should I stop injecting into that area?
Ask the person who prescribed it or a pharmacist: they can look at the area, which is the whole difference between an answer and a guess. What you can bring them is where and when you have injected.
How common are these reactions, really?
In the trials the labels report, 0.2% for Ozempic® and 1.4% for Wegovy®: with 1% of the placebo group reporting them too. Rates differ by product and by what the product was being trialled for, so a number from one label is not a number for another.
